    Note for staff: This form is to be completed when a tenant is in breach of their contract/tenancy. The from will send warning emails to the tenant and inform the landlord. Where necessary the police and local authority need to be informed. Please also update the journal and monitor the status of this. If the issue is NOT resolved after the due date, repeat process with the next warning. If after the final warning the issue is not resolved, inform a senior member of staff who will advise on next steps i.e. notice of seeking possession of the property and eviction.
